Thank you for choosing Carolina Home Mortgage.  The documents listed below are necessary for the approval of your loan application.  Please e-mail, fax, or mail these documents to your Carolina Home Mortgage Loan Originator.

• Copy of your driver's license(s)
• Most recent month's bank statement for all accounts (all pages)
• Most recent stock, mutual fund, or retirement statements for all acounts (all pages)
• If salaried: Copy of past year's W-2 form(s) and pay stub(s) for the most recent 30 days*
• If self-employed: Copy of past year's personal federal tex return(s) with ALL schedules.  Be sure to sign the second page of the 1040 form*
• Name & telephone number of your closing attorney and homeowner's insurance agent
• A copy of your most recent mortgage statement (refinances only)
• A copy of your purchase contract (purchases only)

*For Government financing (FHA,VA,USDA) please include your 2007 W-2 (or personal federal tax return) as well as 2 months of bank statements
